1. Eligibility

Survey Sampling International Prize Draw (the “Prize Draw”) is open only to legal residents of fifty (50) United States and the District of Columbia, Belgium, Germany, Spain, France, Italy, United Kingdom, Netherlands, Switzerland, Austria, Poland, Canada, Finland, Argentina, Brazil and Ireland who are eligible to participate on a Survey Sampling International Panel ("Panel") in their respective country of residence and meet the minimum age requirement for the respective Panel. Employees of Survey Sampling International LLC, SSI Netherlands BV, ePrize, LLC, and their parent, subsidiary and affiliate companies and suppliers, vendors and agents, as well as their family and household members are not eligible. Subject to all applicable laws and regulations. Void where prohibited. Participation constitutes entrant’s full and unconditional agreement to these Official Rules and Sponsor’s (as defined herein) and Administrator’s (as defined herein) decisions, which are final and binding in all matters related to the Prize Draw. Winning a prize is contingent upon fulfilling all requirements set forth herein.

3. Timing

The Prize Draw begins at 00:00 Greenwich Mean Time ("GMT"), 15 June 2010, and ends at 23:59 GMT, 30 June 2011 (the "Promotion Period"). The Prize Draw consists of five (5) entry periods (each, an "Entry Period") as outlined in the chart below:

4. How to Enter

During the Promotion Period, Sponsor will operate country-specific Panels for which individuals have registered or may register. The Promotion Period consists of five (5) separate Entry Periods. The prizes for which you are eligible depend on your country of residence as set forth in Section 6 below. Limit: New Panelists registering with an applicable Panel during the Promotion Period may receive: one (1) entry into the applicable Entry Period drawing for registering and completing the registration survey, if any. Each eligible entrant may receive: one (1) entry into the applicable Entry Period drawing for completing a survey or receiving a screened out notice or message, or receiving a quota full message regarding a survey, and one (1) entry into the applicable Entry Period drawing for updating his/her profile upon request of the Panel, if any. Each eligible entrant must have his/her own email address, separate from the email address of any other eligible entrant. Any attempt by any entrant to obtain more than the stated number of entries by using multiple/different email addresses, identities, registrations and logins, by joining multiple Panels, or any other methods, will void that participant’s entries and may subject such individual to additional sanctions or penalties in accordance with the applicable Panel Terms and Conditions.
Use of any automated system to participate in the Prize Draw is strictly prohibited and will result in disqualification and all entries submitted or received via such automated system will be void. Neither Sponsor nor Administrator are responsible for lost, late, incomplete, invalid, unintelligible or misdirected information or entries, which will be disqualified and void. In the event of a dispute as to the identity of any entrant, the authorized account holder (as defined herein) of the email address used by, or connected to, the eligible entrant will be deemed to be the entrant. The “authorized account holder” is the natural person assigned an email address by an Internet access provider, online service provider, Internet service provider or other organization responsible for assigning email addresses for the domain associated with the submitted address. Potential winners may be required to show proof of being the authorized account holder.
Mail-In Method of Entry (Only for Residents of the fifty (50) United States and the District of Columbia): To enter without completing a survey, hand print your name, address, day and evening phone numbers, email address (if any), and age on a 3" x 5" piece of paper and mail it with proper postage to “Survey Sampling International Prize Draw - 3,” c/o ePrize, LLC, Dept. 565184, P.O. Box 5046, Kalamazoo MI 49003-5046. You will receive one (1) entry into the applicable Entry Period drawing. Limit: One (1) per envelope. All mail-in entries must be handwritten. All mail-in entries must be postmarked and received by the dates specified in the chart above. All entries become the exclusive property of Sponsor, and none will be acknowledged or returned. Proof of sending or submission will not be deemed to be proof of receipt by Sponsor. Sponsor is not responsible for lost, late, incomplete, invalid, unintelligible, illegible, misdirected or postage-due entries, which will be disqualified.
For Residents of France Only: Upon written request, Administrator will refund to you the expense of your Internet connection incurred to enter the Prize Draw. The amount refunded to you will be €0.50, regardless of the actual expense incurred by you. To request a refund, please send your request, and any necessary justification, to: "Survey Sampling International Prize Draw – 3," c/o ePrize, LLC, One ePrize Drive, Pleasant Ridge, MI 48069, U.S.A. Entrants may be required to provide proof that expenses were incurred to enter the Prize Draw. The postage used in this request may also be refunded upon written request to the same address. Requests must be received by 30 June 2011.
5. Prizes Drawings and Verification of Potential Winners

Winning is contingent upon complying with all terms and conditions of, and fulfilling all requirements contained in, these Official Rules. Administrator, an independent judging organization, will randomly select one (1) potential Prize Draw winner from all eligible entries received from residents of each eligible country during each Entry Period on the dates specified in Section 3 above. Each drawing will take place at the Administrator’s offices at the address set forth above around 17:00 GMT. Entries, whether winning or non-winning, will not be carried over into, and will not be included in any subsequent Entry Period drawings. The potential winners shall be notified either by email, telephone, or registered mail after the date of the Prize Draw drawing. Except where prohibited, each potential winner may be required to sign and return winner documentation, as determined by Sponsor in its sole discretion, to confirm eligibility, release Sponsor and Administrator of liability, or for any other reason deemed necessary by Sponsor. If documentation is required, it must be received by Sponsor or Administrator within the time frame indicated at the time of initial win notification otherwise the potential winner forfeits the prize. The potential winners from Canada will also be required to answer a time-limited skill testing question to be eligible to receive a prize. If a potential winner cannot be contacted, fails to sign and return the required documentation (if applicable), does not answer the skill-testing question correctly (if applicable), or prize is returned as undeliverable, the potential winner forfeits the prize. Potential winners must continue to comply with all terms and conditions of these Official Rules, and winning is contingent upon fulfilling all requirements. In the event that a potential winner is disqualified for any reason, Administrator will award the applicable prize to an alternate winner by random drawing from among all remaining eligible entries from the applicable country. Only three (3) alternate drawings will be held, after which the prize will remain un-awarded the. Prizes will be fulfilled approximately 10-12 weeks after winner verification. Any un-awarded prize(s) will not be combined with the prize(s) for, or otherwise carried over to, any subsequent Entry Period.

a. United States Prizes: FIVE (5) UNITED STATES PRIZES (one (1) per Entry Period): $12,000 USD. Total Value of United States Prizes: $60,000 USD.
b. Belgium Prizes: FIVE (5) BELGIUM PRIZES (one (1) per Entry Period): €1,000. Total Value of Belgium Prizes: €5,000.
c. Germany Prizes: FIVE (5) GERMANY PRIZES (one (1) per Entry Period): €2,500. Total Value of Germany Prizes: €12,500.
d. Spain Prizes: FIVE (5) SPAIN PRIZES (one (1) per Entry Period): €2,000. Total Value of Spain Prizes: €10,000.
e. France Prizes: FIVE (5) FRANCE PRIZES (one (1) per Entry Period): €2,500. Total Value of France Prizes: €12,500.
f. Italy Prizes: FIVE (5) ITALY PRIZES (one (1) per Entry Period): €2,000. Total Value of Italy Prizes: €10,000.
g. United Kingdom Prizes: FIVE (5) UNITED KINGDOM PRIZES (one (1) per Entry Period): £5,000. Total Value of United Kingdom Prizes: £25,000.
h. Netherlands Prizes: FIVE (5) NETHERLANDS PRIZES (one (1) per Entry Period): €2,000. Total Value of Netherlands Prizes: €10,000.
i. Switzerland Prizes: FIVE (5) SWITZERLAND PRIZES (one (1) per Entry Period): 1,000 CHF. Total Value of Switzerland Prizes: 5,000 CHF.
j. Austria Prizes: FIVE (5) AUSTRIA PRIZES (one (1) per Entry Period): €1,000. Total Value of Austria Prizes: €5,000.
k. Poland Prizes: FIVE (5) POLAND PRIZES (one (1) per Entry Period): zł6,000. Total Value of Poland Prizes: zł30,000.
l. Canada Prizes: FIVE (5) CANADA PRIZES (one (1) per Entry Period): $1,500 CND. Total Value of Canada Prizes: $7,500 CND.
m. Finland Prizes: FIVE (5) FINLAND PRIZES (one (1) per Entry Period): €1,000. Total Value of Finland Prizes: €5,000.
n. Argentina Prizes: FIVE (5) ARGENTINA PRIZES (one (1) per Entry Period): $5,800 ARS. Total Value of Argentina Prizes: $29,000 ARS.
o. Brazil Prizes: FIVE (5) BRAZIL PRIZES (one (1) per Entry Period): R$2,600. Total Value of Brazil Prizes: R$13,000.
p. Ireland Prizes: FIVE (5) IRELAND PRIZES (one (1) per Entry Period): €1,000. Total Value of Ireland Prizes: €5,000.
For All Prizes: Prizes will be awarded as cash, or a cash equivalent. Winners may be required to set up a money transfer account, as directed by Sponsor, in order to receive his/her prize. Prizes are non-transferable and no substitution will be made except as provided herein at the Sponsor’s sole discretion. Sponsor reserves the right to substitute a prize for one of equal or greater value if the designated prize should become unavailable for any reason. Neither Sponsor nor Administrator shall be liable or responsible for any lost, stolen, or misplaced prizes. Winners are responsible for all taxes and fees associated with prize receipt and/or use. Odds of winning a prize depend on the number of eligible entries received from Panel members residing within each eligible country during each Entry Period. Limit: One (1) prize per person, per Entry Period.
7. Release

By entering the Prize Draw, each entrant agrees to release and hold harmless Sponsor, SSI Netherlands BV, Administrator, and their respective subsidiaries, affiliates, suppliers, distributors, advertising/promotion agencies, and prize suppliers, and each of their respective parent companies and each such company’s officers, directors, employees and agents (collectively, the “Released Parties”) from and against any claim or cause of action, including, but not limited to, personal injury, death, or damage to or loss of property, arising out of participation in the Prize Draw or receipt or use or misuse of any prize. This Release does not apply to claims for personal injury or death caused by the negligence of the Released Parties.
8. Publicity

Except where prohibited, participation in the Prize Draw constitutes winner’s consent to Sponsor’s and its agents’ use of winner’s name, likeness, photograph, voice, opinions and/or hometown and state for promotional purposes in any media, worldwide, without further payment or consideration.
9. General Conditions

Sponsor reserves the right to cancel, suspend and/or modify the Prize Draw, or any part of it, if any fraud, technical failures or any other factor beyond Sponsor’s reasonable control impairs the integrity or proper functioning of the Prize Draw or, as determined by Sponsor in its sole discretion and, in regards to the performance of the Prize Draw or any part thereof in Quebec, Canada, subject to the approval of the Régie des alcools, des courses et des jeux. Sponsor reserves the right in its sole discretion to disqualify any individual it finds to be tampering with the entry process or the operation of the Prize Draw or to be acting in violation of these Official Rules or in an unsportsmanlike or disruptive manner. Any attempt by any person to deliberately undermine the legitimate operation of the Prize Draw may be a violation of criminal and civil law, and, should such an attempt be made, Sponsor reserves the right to seek damages from any such person to the fullest extent permitted by law. Sponsor’s failure to enforce any term of these Official Rules shall not constitute a waiver of that provision.
10. Limitations of Liability

The Released Parties are neither liable or responsible for: (1) any incorrect or inaccurate information, whether caused by entrants, printing errors or by any of the equipment or programming associated with or utilized in the Prize Draw; (2) technical failures of any kind, including, but not limited to malfunctions, interruptions, or disconnections in phone lines or network hardware or software; (3) unauthorized human intervention in any part of the entry process or the Prize Draw; (4) technical or human error which may occur in the administration of the Prize Draw or the processing of entries; (5) late, lost, undeliverable, damaged or stolen mail; or (6) any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from entrant’s participation in the Prize Draw or receipt or use or misuse of any prize. If for any reason an entrant's entry is confirmed to have been erroneously deleted, lost, or otherwise destroyed or corrupted, entrant’s sole remedy is another entry in the Prize Draw, provided that, if it is not possible to award another entry due to discontinuance of the Prize Draw, or any part of it, for any reason, Sponsor, in its sole discretion, may elect to hold a random drawing from among all eligible entries received up to the date of discontinuance for any or all of the prizes offered herein. No more than the stated number of prizes will be awarded. In the event that production, technical, seeding, programming or any other reasons cause more than the stated number of prizes as set forth in these Official Rules to be available and/or claimed, Sponsor reserves the right to award only the stated number of prizes by a random drawing among all legitimate, un-awarded, eligible prize claims.
11. Disputes

Except where prohibited, entrant agrees that: (1) any and all disputes, claims and causes of action arising out of or connected with this Prize Draw or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the United States District Court for the Eastern District of Michigan (Southern Division) or the appropriate Michigan State Court located in Oakland County, Michigan, U.S.A.; (2) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering this Prize Draw, but in no event attorneys’ fees; and (3) UNDER NO CIRCUMSTANCES WILL ENTRANT BE PERMITTED TO OBTAIN AWARDS FOR, AND ENTRANT HEREBY WAIVES ALL RIGHTS TO CLAIM, INDIRECT, PUNITIVE, INCIDENTAL AND CONSEQUENTIAL DAMAGES AND ANY OTHER DAMAGES, OTHER THAN FOR ACTUAL OUT-OF-POCKET EXPENSES, AND ANY AND ALL RIGHTS TO HAVE DAMAGES MULTIPLIED OR OTHERWISE INCREASED. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of the entrant and Sponsor in connection with the Prize Draw, shall be governed by, and construed in accordance with, the laws of the State of Michigan, U.S.A. without giving effect to any choice of law or conflict of law rules (whether of the State of Michigan, U.S.A. or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than the State of Michigan, U.S.A.
Residents of Quebec Only: Any litigation respecting the conduct or organization of this Prize Draw in Quebec, Canada may be submitted to the Régie des alcools, des courses et des jeux for a ruling. Any litigation respecting the awarding of a prize may be submitted to the Régie only for the purpose of helping the parties reach a settlement.

Personal information will be used for the purpose(s) for which it is disclosed, including, without limitation, membership in a Panel; and eligibility for, and communication in connection with, the Prize Draw. Entrants have the right to access, withdraw, and correct their personal information. In regards to accessing, withdrawing, and/or correcting personal information to be used in connection with the Prize Draw, Entrants may request such action by sending a message to "Survey Sampling International Prize Draw - 3" Opt-out Request c/o ePrize, LLC, One ePrize Drive, Pleasant Ridge, MI 48069 U.S.A.
The personal information submitted, received, or collected in connection with the Prize Draw shall be stored on a server within the United States of America. Entry into the Prize Draw constitutes your acknowledgement of, and explicit consent to, the export of your personal information to the United States of America and its use there for the purposes of administering this Prize Draw and associated processing (e.g., but not limited to back-up, archiving). This server is controlled by the Sponsor and/or the Administrator who have notified the US Department of Commerce that they adhere by the rules of the Safe Harbor Framework available under http://www.export.gov/safeharbor/eg_main_018236.asp.
13. Winner List

Winner List requests will only be accepted after the promotion end date (listed above) and no later than 31 October 2011. For the Winner List, send an email with subject line: "Survey Sampling Membership Prize Draw – 3, Winner List Request," to WinListRequests@eprizefulfillment.com.
14. For Residents of France Only

Pursuant to French law pertaining to data collection and processing, you have a right of access to, modification and withdrawal of your personal data. You also have the right of opposition to the data collection, under certain circumstances.
